  I would recommend a masthead amplifier over this, which is what I use.

  Can this be used internally and at the TV end or outside only?

  Internally. 
  
  I have mine in the loft, cable tied to one of the roof timbers and plugged into the aerial. 
  You just plug the power supply in by your TV and it is job done. 
  
  Another benefit is this will improve all your aerial outlets, not just the one it's plugged into.

  I only use the aerial in the loft for one television, cable runs from the loft aerial down back of wall which ultimately comes from the wall socket.
  
  As this booster is a 23db gain I wonder if any that are less than 23db would help such as 20db gain? 
  
  I saw one at gain 20db and as I currently have 100% for both signal strength and quality I'm therefore unable to know how much over 100% the quality and strength are?
